The MedsCheck program provides a medical review of your medication. Individuals are provided with a one-on-one interview with a pharmacist. You will review your prescription and non-prescription medication needs. The purpose of this review is to help people better understand how their medication works, if it is taken as prescribed, and if they are getting the most benefits out of it.
There are a number of eligibility requirements in order to qualify for a MedsCheck appointment. You must be a resident of Ontario and have a valid Ontario Health Card. The program is voluntary, but patients must agree to use the service. Qualifying patients are eligible for one MedsCheck annual per year. Some patients are eligible for a follow up if they have recently been discharged from the hospital, have received a referral from a doctor, or because of a pharmacists documented decision.
Ontario funds the MedsCheck program for:
- Anyone who takes three or more prescription medications for an ongoing, chronic condition
- Anyone diagnosed with either type 1 or type 2 diabetes (even if you are not taking diabetes medication)
- Any resident of a long-term care home
No. MedsCheck consultations are available to eligible Ontarians free of charge. The program is paid for by the Ontario Government through the Ontario Drug Benefit (ODB) Health Network System.
Make sure you bring the following to your MedsCheck appointment:
Please ensure the clinical pharmacist can access your current MAR (Medication Administration Record) sheets and access the medications that each individual is taking. It is helpful for the pharmacist to also have access to each individual medical history.
We typically allot for 20 to 30 minutes per individual MedsCheck. This is the standard length of an appointment. However, some appointments may take more or less time depending on your specific situation.
Booking an appointment with one of our Resident Care pharmacists is simple. A Pharmacy team member will contact your homes manager/supervisor to book a annual visit from our clinical pharmacist to conduct Medschecks for all eligible individual’s.
After the MedsCheck appointment the staff or individual and the pharmacist will sign and date the MedsCheck personal medication record (a list of all medications) a copy of this record will be provided to you after the consultation and after the pharmacist has resolved any drug therapy issues. This may be immediately after the consultation if there are no issues or when the pharmacist arranges for the record to be provided. The completed MedsCheck record is signed and dated by the pharmacist and is also shared with the primary care provider.
